Your First Appointment:
The first appointment will include a detailed medical history, a physical examination and an assessment of your legs and feet by a registered foot specialist / chiropodist. We require all patients to fill out an initial intake form prior to the first visit. For your convenience, the new patient intake form is provided below. You may fill the form and email it to [email protected] or bring it with you to the appointment if possible. If you are unable to, please arrive 10 minutes earlier to complete the form.

Please bring the following items:
- Completed initial intake form (if possible)
- List of current medications
- Any referrals (if applicable)
- Previously taken X-rays/X-ray reports (If you have a copy)
- Your most commonly worn footwear.
- List of any questions/concerns you may want to discuss with the practitioner.
- If applicable, bring details of current insurance provider (name of insurer, type of insurance, policy #, member ID etc.)

Do I need a medical referral TO SEE A FOOT SPECIALIST / Chiropodist ?
No, foot specialists / chiropodists are primary care practitioners and therefore do not require medical referrals; however medical referrals are welcome. If you or someone you know needs help with their foot concerns – they are more than welcome to visit the Halton Foot & Orthotic Clinic.
Are foot care services covered by OHIP?
Foot care services in Ontario are not covered under the Ontario Health Insurance Plan (OHIP); however, most private/extended health insurances, NIHB, ODSP, WSIB, veteran's affair etc., help cover your visit costs. Itemized receipts along with all supporting documentations needed to submit your claim will be provided.

What is the difference between a chiropodist and podiatrist?
Foot specialists were originally titled as Chiropodists. In 1950, schools in the USA decided to use a more known and used term of "Podiatry"; while Ontario remains to be the only place in the world using the old legislative term of "Chiropody". Essentially, a Chiropodist is Canadian trained and a Podiatrist is American trained. Both chiropodists and podiatrists can assess foot & ankle problems, prescribe medications, perform injections, perform soft tissue surgery and prescribe orthotics/orthopaedic footwear.
